Key Responsibilities

• Conduct thorough assessments of patients' physical functioning, mobility, and overall health to develop and implement individualized treatment plans.

• Educate patients and their families on therapeutic exercises, the use of assistive devices, and strategies to promote ongoing health outside of therapy sessions.
• Coordinate with physicians and the patient care team to ensure comprehensive management of therapeutic services, making adjustments to care plans as necessary.
• Maintain accurate documentation of patient progress, treatment modalities used, and outcomes achieved to facilitate communication within the healthcare team.
• Prioritize and manage your caseload effectively to ensure the efficient delivery of therapy while assessing daily staffing requirements.

Patient Population / Client Focus
