Work Environment

This position will operate onsite, delivering physical therapy services within the comfort of patients' homes. This unique setting allows you to create customized treatment plans that align with each patient's living environment, enhancing therapeutic outcomes and personal comfort. Your weekdays will typically consist of working Monday through Friday, from 9 AM to 5 PM, ensuring a balanced work-life dynamic while serving our community.

Key Responsibilities

• Conduct thorough assessments of patient conditions to accurately identify physical challenges and rehabilitative needs.
• Develop and implement personalized therapy plans, incorporating evidence-based practices that target improved mobility and function.
• Utilize specialized therapeutic techniques, such as manual therapy, exercise prescription, and modalities relevant to home care settings, ensuring effective treatment delivery.
• Collaborate consistently with a diverse healthcare team, including physicians, nurses, and other health professionals to coordinate holistic care efforts.
• Provide education and support to patients and their families on care plans, rehabilitation techniques, and preventive measures to ensure continuity of care beyond therapy sessions.
• Keep accurate, detailed records of patient progress, treatment plans, and outcomes using electronic medical record systems, enabling data-driven decisions in patient care.

Patient Population / Client Focus

Your primary focus will be elderly patients and individuals recovering from surgeries, chronic illnesses, or injuries who seek rehabilitative support in their home environments. You'll be instrumental in promoting independence, enhancing mobility, and reducing fall risks among this patient population through tailored interventions and support strategies.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid Physical Therapist license in the state of California.
• Comprehensive knowledge of human anatomy, physiology, and the principles of physical therapy practices.

Compensation and Benefits
