Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of hospice patients, identifying their unique emotional, social, and spiritual needs in order to inform individualized care plans.

• Provide face-to-face and virtual counseling services aimed at helping patients and their families cope with the mental and emotional challenges associated with terminal illness.

• Collaborate with interdisciplinary teams to devise and implement personalized care strategies that encompass behavioral modification, stress management, and end-of-life transitions.

• Facilitate support groups for patients and caregivers that foster open communication and shared experiences, focusing on mental health and emotional well-being.

• Document patient interactions, including assessments and treatment plans, accurately and promptly in the electronic health record (EHR) system, ensuring continuity of care and compliance with regulations.

• Engage in regular outreach to patients and families to promote ongoing support, enhancing engagement with mental health resources available through the hospice organization.

Patient Population / Client Focus\n

In this role, you will primarily serve patients within the hospice setting, providing support to those facing terminal illnesses and their families. You will work with a diverse patient population, understanding their cultural backgrounds, spiritual beliefs, and specific psychological needs as they navigate the complexities of end-of-life care.

Qualifications\n

• Education: A Master's degree in Social Work from an accredited program.

• Licensure: An active state licensure as a Licensed Clinical Social Worker (LCSW) is required.

• Experience: A minimum of 3 years of experience in a mental health or outpatient setting, ideally with exposure to hospice or palliative care.

Compensation and Benefits\n
