Key Responsibilities\n

In this role, you will be responsible for:

• Assisting mental health consumers in navigating both the physical and behavioral health care systems to facilitate integrated health care solutions.

• Educating clients on available resources and services to encourage their active participation in disease prevention, wellness promotion, and strategies for achieving a healthy lifestyle.

• Engaging in multidisciplinary treatment team meetings to collaboratively develop and implement individualized treatment plans.

• Maintaining thorough and accurate clinical documentation for all client interactions, ensuring compliance with organizational standards and legal requirements.

• Administering and interpreting evidence-based screening tools to assess the healthcare needs of individuals, then reporting findings to the treatment team for optimal outcomes.

Your work will involve a diverse patient population, including individuals dealing with anxiety, depression, trauma, and other mental health issues. Qualifications\n

• Master’s degree in Professional Counseling from an accredited program.

• Current licensure as a Licensed Professional Counselor (LPC) in Pennsylvania.

• A minimum of three years of relevant clinical experience in a behavioral health setting.
