Description

• Conduct patient evaluations in inpatient or outpatient settings and create personalized treatment plans
• Implement physical therapy interventions focusing on mobility, strength, balance, and functional outcomes
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Instruct patients and their families on therapeutic objectives, discharge planning, and at-home exercise protocols
• Document all evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to guarantee comprehensive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited program

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional certifications (e.g., neurological, geriatric specialties) are advantageous but not necessary

• Prior experience strongly preferred
