About the Role: As a Licensed Social Worker, you will provide assessment, counseling, and support to our patients and residents to help them achieve their goals and improve their overall well-being. You will work closely with our interdisciplinary team to develop and implement individualized care plans that address the physical, emotional, and social needs of our patients and residents. You will also provide education and support to their families and caregivers to help them navigate the healthcare system and provide continued support after discharge.
