Key Responsibilities\n

• Help clients and their families navigate the emotional challenges associated with their health conditions through therapeutic counseling and emotional support.

• Understand and advocate for patient rights, ensuring ethical practices and decision-making are upheld throughout the therapeutic process.

• Respond appropriately to vulnerabilities or suspicions of abuse in accordance with legal requirements.
\n
• Collaborate in care system initiatives that aim to reduce hospital readmissions, enhancing continuity of care.

• Lead transition conferences that facilitate smooth care handoffs between various healthcare providers and the client’s family.
\n
• Employ advanced tools and technology to identify patients at risk of readmission and intervene promptly.
\n
• Utilize the electronic medical record system to document client progress, monitor therapeutic outcomes, and adjust care plans as needed.
\n
• Actively participate in interdisciplinary team meetings to coordinate care and ensure that the treatment plan addresses the holistic needs of the patient.

• Provide comprehensive support and resources to clients, addressing their specific financial and social needs through effective case management.
\n
• Continuously update knowledge of community resources and payer networks to facilitate appropriate care transitions for patients.

• Other tasks as assigned in the pursuit of providing exceptional mental health care.

Patient Population / Client Focus\n

You will primarily serve individuals facing significant emotional distress due to acute, chronic, or terminal health issues, along with their families. Your role will focus on providing not only therapeutic interventions but also education and connection to community resources that promote healing and resilience. Qualifications\n

• A valid independent license in the state of Oregon (LCSW, LMFT, LPC) is required.

Compensation and Benefits\n
