Key Responsibilities\n

• Conduct thorough psychosocial assessments by utilizing interviews, observations, and medical record reviews to evaluate patients’ emotional, social, and environmental factors that impact their recovery journey.

• Provide individual and family counseling services to patients, focusing on their adjustment issues, emotional well-being, and coping strategies for substance use challenges.

• Collaborate closely with a multidisciplinary rehabilitation team to create individualized treatment plans tailored to meet each patient’s unique needs and goals.

• Assist patients in navigating community resources such as support groups, vocational rehabilitation, and housing assistance, ensuring they have access to the support they need during recovery.

• Educate patients and their families about available community resources and post-rehabilitation services to promote an effective transition back into everyday life.

• Engage in discharge planning to ensure continuity of care and successful reintegration into the community after detox treatment.

Patient Population / Client Focus\n

Your primary focus will be working with individuals dealing with substance use disorders at the detox stage of their recovery. Qualifications\n

• Must hold a valid Licensed Clinical Social Worker (LCSW) license in California.
