Key Responsibilities\n

• Perform ongoing assessments of clients to identify their mental health needs and develop tailored treatment plans.

• Establish effective communication channels with supervisors, direct care staff, and families to update them on clients' progress.

• Create and maintain a structured schedule for appointments, home visits, and necessary observations to track client advancements.
\n
• Conduct home visits when necessary to observe clients in their environment, perform chart reviews, and gather data through interviews.
\n
• Oversee the implementation of treatment program plans, ensuring techniques are applied effectively for the benefit of clients.

• Participate in Interdisciplinary Team Meetings, contributing your expertise to collaborative discussions about client care strategies.

• Document monthly and quarterly progress notes to maintain accurate records of client progress and treatment adjustments.
\n
• Organize training and training materials to enhance staff competencies regarding program implementation and documentation practices.
\n
• Conduct follow-up interviews and additional home visits to provide ongoing support consistent with clients’ evolving needs.

• Serve as a liaison with the public and mental health community, promoting collaborative initiatives to meet developmental goals.

• Assist in developing procedures aimed at refining training materials and enhancing overall program effectiveness.

Patient Population / Client Focus\n

The clients you will work with encompass a diverse demographic, including both adults and youth facing various mental health challenges. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related discipline.

Compensation and Benefits\n
