Key Responsibilities\n

• Assist primary care providers by identifying and addressing mental health and psychosocial issues, contributing actively to the multidisciplinary primary care team.

• Conduct comprehensive patient intakes that include diagnostic assessments and functional evaluations to inform treatment goals and plans.
\n
• Offer brief, focused interventions for clients requiring immediate mental health support, ensuring effective management of their individual needs.
\n
• Provide recommendations to primary care providers on the necessity of referrals for specialty mental health services when appropriate.
\n
• Initiate and manage follow-up with clients to monitor their progress and determine if adjustments to treatment strategies are required.
\n
• Develop relapse prevention plans for clients to support their ongoing stability and mental wellness.
\n
• Detect at-risk clients and develop proactive plans aimed at preventing deterioration of their conditions.
\n
• Coordinate health service delivery related to behavioral care, including collaboration with other treatment providers, with patient consent.
\n
• Maintain accurate and up-to-date documentation of patient evaluations, progress notes, and treatment plans in compliance with telehealth best practices.
\n
• Work alongside primary care providers to inform them of client needs and collaboratively create comprehensive treatment plans.
\n
• Empower clients to make healthier lifestyle changes and equip them with self-management skills to facilitate their own treatment and recovery processes.
\n
• Provide behavioral health consultations to clinic management and support the improvement of program evaluation and outcomes.
\n
• Assist clients in adhering to medical treatments prescribed by primary care providers, including strategies to manage potential side effects of medications.
\n
• Adhere to the Code of Ethical Conduct for Clinical Professionals while performing all duties.
\n
• Perform additional tasks as assigned by leadership.

Patient Population / Client Focus\n

The primary focus of your role will be to support veterans and their families, who may be facing a variety of mental health challenges. You will engage with individuals from diverse backgrounds, each requiring tailored intervention strategies to enhance their emotional well-being and recovery journey. Our clients often deal with the complexities associated with military service, including PTSD, depression, anxiety, and other psychosocial challenges.

Qualifications\n

• A Master’s degree in Clinical Counseling or a related field is required.

• Must possess a valid Licensed Professional Counselor (LPC) license.

• Solid understanding of mental health assessment, treatment planning, and current psychotherapeutic methodologies.

• Proficiency in English is required; bilingual capabilities in Spanish are preferred but not mandatory.

• Demonstrated mental and physical health to effectively meet the demands of this role.

• Comprehensive knowledge of the impact of mental health issues on individuals, families, and the wider community.

• Familiarity with local community resources and the healthcare system.
\n
• Experience in psycho-diagnostics and psychotherapy techniques.

• Strong ability to assess individual client needs and evaluate the effectiveness of care provided.

Preferred Experience\n

• Experience working within a Federally Qualified Health Center (FQHC) setting is a plus.

• Prior experience serving veterans or individuals in crisis will be highly regarded.
