RN Homecare Team Leader - Queens NY

RN Team Leader Salary: $106,000 - $125,300 per year. Job Details We are looking to hire an experienced RN with BSN degree and active New York State RN License in good standing who comes with at least 3 years homecare experience to serve in an RN Team Leader role. This full‑time position will function with leadership administrative duties, oversee a team of homecare nurses, precept new nurses, supervise peers on field visits, conduct supervisory home visits with nursing staff to ensure clinical competence, provide guidance in caring for patients with various diagnoses, and address patient/family concerns. In this role, you will also carry a very light patient caseload, coordinate, implement and continually evaluate the home care needs of your patients, review patient insurance information and create and implement a plan of care, assess the patient for the appropriate level of care and utilization of agency resources, perform comprehensive assessments, verify the plan of care with physicians, ensure EMR accuracy, complete OASIS data sets, and perform medication reconciliation. Responsibilities Carry a very light patient caseload. Coordinate, implement, and continually evaluate the home care needs of patients. Review patient insurance information and create and implement a plan of care. Assess the patient for the appropriate level of care and utilization of agency resources. Perform a comprehensive physical, psychosocial and safety assessment via examination, observation and motivational interviewing. Verify the initial plan of care with the patient’s physician; inform the physician of any changes in the patient’s status; update the plan of care accordingly. Ensure the EMR accurately reflects the current plan of care, services provided, patient’s response to treatment, and status of goal achievement. Accurately complete all OASIS data sets within agency established guidelines, reflecting positive clinical outcomes and consistent utilization of industry established best practices. Perform comprehensive medication reconciliation, including all physician prescribed and OTC medications and/or supplements.
