Job Summary The Physical Therapist Home Health collaborates with the interdisciplinary team to deliver high‑quality physical therapy services in patients’ homes. Responsibilities Perform initial evaluation to identify functional deficits and establish a Plan of Treatment in consultation with the physician, including short‑ and long‑term goals. Provide treatment to relieve pain, develop or restore functions, and maintain maximum performance using therapeutic exercises, massage, dry and moist heat, and other modalities. Direct and aid patients in active and passive exercises, muscle re‑education, gait and functional training, activities of daily living, transfer activities, and prosthetic training. Use modalities such as low‑voltage generators, ultrasonic machines, heat and cold. Arrange outpatient therapy services for procedures that cannot be completed at home. Observe, record, and report patient responses and changes in condition to the physician. Communicate with the Rehab Program Manager, Patient Care Manager, Clinical Coordinator and appropriate staff to plan total patient and family care. Assist in discharge planning as appropriate. Supervise Physical Therapy Assistants as required by regulation. Instruct other health team personnel during appropriate phases of physical therapy. Instruct patients in the use of wheelchairs, braces, crutches, walkers, canes, and prosthetic and orthotic devices. Facilitate provision of equipment as needed. Provide orientation and supervision in home‑care physical therapy skills to PT students and new orientees. Qualifications Graduate of a physical therapy program approved by the Council on Medical Education of the American Medical Association in collaboration with the American PT Association or its equivalent, and by the State required for licensure. Licensed Physical Therapist in Pennsylvania or New Jersey. One to two years of experience under a qualified registered Physical Therapy Supervisor preferred. CPR certification required. Physical and Sensory Requirements Ability to sit for up to 90 minutes and stand for 30–45 minutes consecutively. Ability to use hands and fingers to manipulate dials on machines. Ability to lift or move patients up to 300 lbs and push/pull patients weighing up to 300 lbs. Ability to stoop, bend, squat, crouch, kneel, and reach above shoulder level frequently. Capacity for tactile sensation to feel muscle spasms, texture, and skin temperature. Ability to hear normal conversation and call bells. General vision, depth perception, and peripheral vision sufficient for job duties. Vehicle Requirements Valid Pennsylvania or New Jersey driver’s license and good driving record accepted by the Network insurance carrier. Motor vehicle insurance with the minimum requirements established by PA or NJ. Compliance with all motor vehicle regulations and laws while operating a vehicle. Routine maintenance of the vehicle.
