Key Responsibilities\n

• Provide therapeutic services to clients in our outpatient clinic, implementing evidence-based interventions tailored to individual needs.

• Conduct comprehensive psychosocial assessments to develop and implement personalized treatment plans in collaboration with the healthcare team.

• Maintain accurate clinical documentation, including progress notes, treatment plans, and discharge summaries, ensuring compliance with departmental policies.

• Serve as a liaison between clients, families, and the healthcare team, facilitating communication regarding treatment goals and progress.

• Execute crisis interventions and manage acute emotional and behavioral issues, ensuring the safety and well-being of clients during critical moments.

• Assist in discharge planning and care coordination, liaising with community resources and service providers to ensure continued support for clients post-treatment.

Qualifications\n

• A Master’s degree in Social Work or Counseling is required.
