Key Responsibilities\n

• Conduct comprehensive assessments and individual therapy sessions tailored to the unique needs of hospice patients, using evidence-based practices such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT).

• Facilitate group therapy sessions that encourage peer support among patients and families, fostering a community of understanding and shared experiences.

• Provide family counseling sessions to assist families in managing complex emotions during end-of-life transitions, equipping them with coping strategies.

• Collaborate closely with a multidisciplinary team of healthcare professionals, including physicians, nurses, and other social workers, to develop and execute personalized care plans for patients.

• Respond promptly to crises, providing necessary interventions and referring clients to appropriate community resources when additional support is needed.

• Ensure meticulous documentation of all client interactions, treatment plans, and progress notes in alignment with HIPAA regulations to maintain patient confidentiality and quality of care.

Qualifications\n

Compensation and Benefits\n
