Work Environment\n

This position allows for a flexible remote work environment, ideally suited for someone who is self-motivated and thrives in an independent setting. While maintaining the warmth and connection typical of in-person counseling, you will leverage digital communication tools and platforms to provide therapeutic support and maintain client records. The work is conducted in conjunction with a multidisciplinary team, including clinical directors and fellow social workers, ensuring coordinated care for each client.

Key Responsibilities\n

• Conduct thorough assessments of client needs, utilizing evidence-based tools to evaluate mental health and substance abuse issues.

• Utilize the DSM criteria for diagnosing substance abuse and determine appropriate care levels according to ASAM guidelines.

• Collaborate in forming individualized treatment plans, specifically crafting the substance abuse counseling components aligned with client goals.

• Design discharge planning and continuum of care strategies to facilitate smooth transitions across various stages of treatment.

• Document all counseling sessions, assessments, referrals, and client interactions in compliance with HIPAA regulations and organizational policy.

• Maintain an active caseload, ensuring regular updates on case management and treatment planning for optimal clinical outcomes.

• Provide support coordination for clients and their families, recording all services provided for effective follow-up care.

• Actively collaborate with external referral sources such as schools, legal entities, and employers to create a supportive network for clients.

• Facilitate interventions that address urgent crisis situations using a multi-disciplinary approach to ensure immediate and effective care.

Patient Population / Client Focus\n

The primary patient population consists of individuals facing significant life challenges due to advanced medical conditions. Qualifications\n

• A Master’s degree in Social Work from an accredited institution and active LCSW licensure is mandatory.

Compensation and Benefits\n

We value the contributions of our team and offer a competitive salary ranging from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
