Key Responsibilities\n

• Provide direct clinical services, including assessment, individual therapy, and group therapy, to clients experiencing major mental illnesses or severe emotional disorders.

• Collaborate with healthcare professionals to design and implement integrated care plans tailored to individual client needs, ensuring seamless access to linked services.

• Conduct evidence-based screenings and assessments to develop appropriate treatment strategies for clients.
\n
• Participate actively in multidisciplinary team meetings to discuss client care, progress, and share best practices aimed at improving outcomes.

• Facilitate crisis intervention efforts as required, ensuring clients receive immediate and effective support during emergencies.

• Maintain accurate documentation of client interactions and treatment plans, ensuring compliance with professional ethical standards and organizational protocols.

• Monitor and evaluate the progress of clients regularly, developing new strategies to enhance their treatment and recovery processes.

• Serve as an advocate for clients, connecting them with community resources and support systems to aid in their recovery journey.

• Participate in committees focused on improving service delivery and exploring innovative approaches to care.

• Provide on-call support during and after regular program hours as assigned.

Your primary focus will be on patients in hospice care, many of whom face significant emotional turmoil and mental health issues related to end-of-life concerns. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid Illinois LCSW license.

Compensation and Benefits\n
