In this role, you'll be the vital link between our clients, their families, and our caregiving staff — ensuring every person we serve receives the highest quality of care. If you're passionate about making a difference in people's lives, this is the opportunity for you!Coordinate and manage home care services for clients, ensuring seamless care deliveryServe as the primary point of contact for clients, families, and caregiversConduct client intake assessments and develop personalized care plansSchedule and match caregivers to clients based on needs, availability, and compatibilityMonitor client satisfaction and proactively address concerns or changes in care needsMaintain accurate client and caregiver records in compliance with home health care regulationsCollaborate with healthcare professionals and referral sources to support continuity of carePrior experience in home health care, case management, or a related healthcare coordination roleStrong interpersonal and communication skills with a client-first mindsetExcellent organizational and multitasking abilities in a fast-paced environmentProficiency with scheduling software and basic computer applicationsKnowledge of home care regulations and compliance standards preferredEmpathetic, dependable, and solutions-oriented team playerHigh school diploma required; Associate's or Bachelor's degree in healthcare or related field a plusComForCare Home Health Care is a trusted provider of in-home care services dedicated to helping clients live their best lives possible.
