Summary: ICC's will create and oversee a process of enhancing supports, building on strengths and meeting needs for referred children and their families, using the wraparound planning process, trauma informed care, and Children's System of Care principles; ICC will partner with CWS, Probation, group home providers, and other local community agencies; average caseload is 10–12 youth. Essential duties and responsibilities include providing assessment, evaluation, plan development, collateral, crisis intervention, and intensive case management services; guiding the wrap team development and process, ensuring smooth transition through the four phases of the wraparound process and emphasizing inclusion of natural supports; developing, coordinating, and ensuring the implementation of the client plan; monitoring data on treatment progress and using this data to modify the client plan as needed; locating, coordinating and developing linguistically and culturally diverse informal and formal resources and networks for families in the communities where they reside; overseeing the development of effective crisis plans, utilizing natural support and, where needed, formal resources, and providing crisis intervention services to families 24 hours a day on on-call; collaborating with CWS social workers, probation officers, and outside resources including schools, psychiatrists, community outreach programs, family advocacy programs, and others as appropriate; participating in fidelity and quality improvement efforts, including wrap coaching, ongoing training, and supervisions; sustaining the team commitment to wraparound values and approach; coordinating efforts with parent and youth partners and other appropriate staff in providing supports and services; maintaining accurate, complete, and timely case records per Medi-Cal requirements and SDCC policies; required to work evenings as assigned to meet the needs of families; fulfilling training requirements including Cerner, cultural competency, HIPAA, and all applicable EBP training; maintaining current licensure and or intern registration; and other duties may be assigned.

Language skills include the ability to read, analyze, and interpret medical records, journals, reports, and legal documents; respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community; effectively present information to management and public groups; and complete timely, organized documentation for trainings.

Reasoning ability includes the ability to define problems, collect data, establish facts, and draw valid conclusions; interpret and deal with several abstracts and concrete variables.

Physical demands include standing, using hands to finger, handle, or feel; reaching with hands and arms; stooping, kneeling, crouching, or crawling; talking and hearing; running, walking, sitting and driving a vehicle; moving and/or lifting a child when clinically necessary; and following a child that AWOLs in a walking or running pace. Specific vision abilities include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Work environment includes moderate noise level.

Qualifications: minimum requirements include a valid California driver's license and reliable transportation, a master's degree from an accredited university, registered associate or in process with the BBS in social work, psychology, professional clinical counseling, or marriage and family therapy and license eligible in the state of California, highly organized with excellent written and verbal communication skills, experience working with high risk youth, and ability to work cohesively on a team. Preferred requirements include proficiency with Smartcare, leadership, public speaking, or group therapy experience, experience working with CWS or Probation youth, experience working in a wraparound program, experience providing training or coaching to adults, and experience providing behavioral modification to children.
