Key Responsibilities\n

• Conduct comprehensive assessments to understand the needs and goals of each patient, tailoring therapeutic approaches accordingly.

• Communicate effectively with supervisors, care team members, family members, and other stakeholders to discuss progress and adjustments to the treatment plan.

• Develop and maintain a detailed schedule of appointments, including follow-ups and home visits, ensuring optimal client engagement and monitoring of treatment progress.

• Organize and conduct home visits or virtual check-ins as necessary to provide comprehensive insights into client behavior and environment.
\n
• Monitor and ensure that each therapeutic program plan is effectively implemented and modified as needed to meet the client's evolving needs.

• Participate actively in team meetings to collaborate on patient care strategies and contribute to the interdisciplinary approach to mental health treatment.

• Document monthly and quarterly progress notes accurately to ensure compliance and continuity of care.
\n
• Facilitate training sessions for staff to enhance their skills in implementing therapeutic plans and documenting interventions.

• Act as a liaison between clients and the broader mental health community, promoting collaboration and support for individual development goals.

• Assist in the creation of training materials and processes aimed at enhancing program effectiveness and staff competency.

Patient Population / Client Focus\n

This role encompasses working with a diverse client base, including children, adolescents, adults, and families facing various mental health challenges. Your expertise will help address issues such as anxiety, depression, trauma, and relationship difficulties, requiring a personal touch to understand and support individual circumstances effectively.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a related field is essential.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) within the state of Illinois.

• A minimum of two years of experience in clinical counseling settings, demonstrating the ability to provide high-quality therapeutic interventions.

• Expertise or focused training in areas such as family therapy, couples therapy, trauma-informed care, or substance misuse treatment will be favorably considered.

• Thorough knowledge of legal and ethical standards governing mental health practice.

Compensation and Benefits\n
