Key Responsibilities\n

In this role, you will be responsible for a range of critical functions:

• Conducting ongoing assessments to develop personalized treatment plans tailored to individual client needs.

• Regularly communicating with supervisors, family members, and the wider mental health community to discuss client progress and adjustments to care.
\n
• Scheduling appointments, home visits, and necessary observations, proactively managing your workload and client needs.
\n
• Coordinating in-home visits to perform observations, gather feedback and carry out comprehensive chart reviews.
\n
• Ensuring the effective implementation of treatment plans and making necessary adjustments to ensure clients receive the best care possible.
\n
• Participating in Interdisciplinary Team Meetings to collaborate with staff on case review and care coordination.
\n
• Documenting progress through detailed monthly and quarterly notes to track client progress and support evidence-based treatment approaches.
\n
• Organizing training sessions to ensure staff and team members are up-to-date with program procedures, fostering a culture of continuous improvement.
\n
• Providing ongoing support through follow-up interviews and home visits based on individual client requirements.
\n
• Maintaining accurate client records to ensure confidentiality and readiness for consultation when required.
\n
• Acting as a liaison with the public and mental health community to promote awareness and support for developmental goals.

• Assisting in the development of training materials aimed at improving program processes and outcomes.

Patient Population / Client Focus\n

Qualifications\n

To be considered for this position, candidates should have:

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• An active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in Illinois.

• At least two years of counseling experience, preferably in a clinical context.

• A strong understanding of legal and ethical standards in mental health practices.
\n
• Excellent communication and interpersonal skills to effectively establish patient rapport.

• Proven ability to work collaboratively within a healthcare team.

• Proficiency in using EMR software and other related technology platforms.

Preferred Experience\n

Additional experience in family therapy, couples therapy, trauma-informed practices, or substance abuse counseling is highly preferred, as is a demonstrated ability to engage with diverse clientele.
