Key Responsibilities\n

• Collaborate closely with a multidisciplinary healthcare team in developing and implementing comprehensive treatment plans. This includes creating actionable discharge plans that are tailored to each patient's psychosocial needs.

• Establish and maintain working relationships with community resource organizations, representing Redesign Counseling Hub in meetings and committees that advocate for the social needs of our patient population.

• Provide individual counseling and crisis intervention to patients experiencing emotional or behavioral difficulties, employing intervention strategies tailored to their specific circumstances.

• Conduct thorough assessments to evaluate patient progress and communicate effectively with physicians and other healthcare professionals regarding any emerging needs or concerns.

• Ensure meticulous documentation of clinical interactions and progress notes in accordance with departmental policies and best practices in social work.

Qualifications\n
