Key Responsibilities

• Conduct thorough evaluations of patients in a home care environment to assess their specific needs and conditions related to recovery from injury, surgery, or chronic illness.
• Develop and implement individualized treatment plans that incorporate various therapeutic methodologies such as manual therapy, therapeutic exercises, and modality applications to enhance recovery and improve mobility.
• Continuously monitor and document patient progress and responses, adjusting treatment plans as necessary to ensure optimal therapeutic outcomes.
• Educate patients, caregivers, and family members on the therapy process, techniques for easing pain, and strategies for enhancing mobility.
• Stay current with advancements in physical therapy by attending workshops and participating in professional development activities; this knowledge is vital in delivering high-quality care.
• Collaborate with other healthcare professionals to facilitate a multidisciplinary approach to patient care and support other team members in achieving the best outcomes.

Patient Population / Client Focus

Our diverse patient population includes children, adults, and seniors recovering from various conditions including post-surgical rehabilitation, sports injuries, chronic pain, and neurological disorders. Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid state license to practice as a Physical Therapist in Virginia.
