Key Responsibilities

• Conduct thorough evaluations of patient needs, which includes comprehensive reviews of medical histories, functional assessments, and the formulation of tailored treatment plans.
• Clearly communicate evaluation findings and collaborate with patients and their families to establish realistic and attainable rehabilitation goals.
• Demonstrate expertise in implementing evidence-based treatment protocols and adjust plans based on ongoing patient evaluations and progress.
• Regularly consult with multidisciplinary teams to ensure coordinated care; recognize significant changes in patient conditions and adapt treatment plans accordingly.
• Manage discharge planning effectively, ensuring comprehensive follow-up care and accurate documentation of all patient interactions, treatment plans, and progress notes.
• Utilize time management skills to maintain an accurate patient schedule while meeting productivity expectations and administrative requirements.
• Participate in departmental initiatives and contribute to achieving departmental goals, ensuring compliance with healthcare regulations and standards.
• Engage in continuing education, training, and mentorship of students and less experienced staff as a way of enhancing professional development and maintaining competency within the organization.
• Exemplify exceptional customer service by being flexible and responsive to patient needs, contributing to a positive rehabilitation experience.

The role focuses on a diverse patient population ranging from post-operative recovery to rehabilitation from chronic conditions, trauma, and sports injuries. Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• Active Physical Therapist license in the State of Virginia; must meet all state regulatory requirements.
• Current Basic Life Support (BLS) certification through the American Heart Association.

Preferred Experience
