Practice Setting: • Hospital Inpatient Requirements: · Board Certification: Yes Neurology · License required: Yes State of OR · Certifications required: TBD · Applicant must provide documentation of provision of surgical services representative of the scope and complexity of the privileges requested during the previous 24 months applicants who completed training during the previous year must provide a training log.
