Responsibilities
- Provide direct patient care to residents in a long-term care setting under supervision
- Administer prescribed medications and treatments in accordance with physician orders and facility policy
- Monitor patient status, document findings in the chart, and communicate changes to the supervising RN
- Assist residents with activities of daily living and accommodate comfort and safety needs
- Collaborate with an interdisciplinary team to support care plans and ensure continuity of care
- Maintain clean, accurate records and uphold infection control and safety standards

Requirements
- Active LPN license in good standing
- Experience in long-term care or skilled nursing settings preferred
- Availability to work the 06:00–18:30 shift on a 3x12-hour schedule
- Willingness to travel for a temporary 12-week assignment starting 10/08/2026
- Strong clinical assessment, documentation, and communication skills
