Responsibilities
- Provide direct, patient-centered care to residents per the plan of care.
- Administer medications and treatments as prescribed; monitor and document responses.
- Monitor vital signs, weight, intake/output, and report changes promptly to the supervising RN.
- Collaborate with RNs, CNAs, and other team members to ensure continuity and quality of care.
- Perform routine procedures such as wound care, dressing changes, and infection control measures.
- Maintain accurate documentation in the chart/EHR and ensure patient safety and privacy.

Requirements
- Active Licensed Practical Nurse license (state of licensure)
- Current Basic Life Support (BLS) certification
- Prior long-term care or geriatric care experience preferred
- Ability to work night shifts and meet guaranteed hours
- Strong communication, teamwork, and documentation skills
- Willingness to undergo standard onboarding requirements (e.g., background check and credential verification)
