The position collects and enters into appropriate databases information relative to a transfer request and develops reports demonstrating program compliance and opportunities for improvement. The position works in collaboration with multiple departments within each hospital to foster relationships with referring facilities, physicians, and hospital staff, this includes the completion of follow-up calls to referral facilities. The position works closely with the Transfer Center Coordinator, Patient Access Coordinator, and CARE leadership to identify, trend, and report barriers to patient throughput. Job Requirements Education Required: High school diploma or equivalent Experience Previous Hospital Or Transfer Center Experience Preferred Skills Excellent interpersonal communication and multitasking Proficient computer skills with Microsoft Office, especially Excel Excellent customer service skills Ability to work with multiple people while meeting numerous requests and deadlines. Goal oriented with initiative and potentials for self-development. Maintains organizational and priority setting skills.
