Care Coordinator

Apply a person-centered approach, keeping clients at the center of program functions Coordinate day-to-day implementation of contractual requirements within state Medicaid guidelines Create care plans based on client needs Train clients on provided services Audit services and report findings to the contracted entity Assist with plan and budget development, submissions, and revisions Provide service to clients, employees, MCOs, and third-party organizations Support daily operations and client advocacy Collaborate with stakeholders regarding service delivery Conduct reporting, oversight, assessments, and client contacts Handle complex service programs and special projects Develop and implement care plans Enroll clients and employees in services, including service/support plans and budgets Orient and educate clients on program roles and responsibilities Train and assist clients with timecard submission Assist clients with program eligibility requirements Travel to client homes or other destinations for client meetings Maintain documentation in the CRM database Compile information for internal or external auditors Market services to referral sources Represent the company at stakeholder, health, and provider fairs Supervise deliverables and ensure timely project completion Participate in professional development and training activities Requirements Bachelor’s degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology, or a related field 6 years’ experience serving individuals with disabilities Must have reliable transportation If driving, a valid driver’s license and auto insurance are required Previous Support Broker experience preferred Combination of education and experience Reliable internet required for remote/hybrid work Must be legally authorized to work in the United States Visa sponsorship and STEM OPT extensions are not provided Bilingual preferred in various locations Core Competencies Demonstrates expertise in client advocacy, care plan development, and compliance with Medicaid guidelines while effectively coordinating services and training clients.
