You will engage with patients and team members through secure electronic health record systems and telehealth platforms, ensuring efficient and effective care delivery. Key Responsibilities Conduct comprehensive mental health assessments to understand patients' psychological and emotional needs, considering any relevant co-occurring medical conditions. Design and execute integrated treatment plans that address both mental health and medical diagnoses, collaborating with other healthcare providers to streamline patient care. Deliver individual and group therapy sessions utilizing evidence-based methodologies that are customized to each patient's situation, promoting therapeutic growth and progress. Provide immediate crisis intervention and stabilization for patients facing acute mental health emergencies through effective communication and problem-solving skills. Educate patients about the interconnectedness of mental health and physical wellness, empowering them to actively participate in their own treatment plans. Maintain meticulous and confidential patient documentation in accordance with HIPAA regulations and ethical standards to ensure legal compliance. Regularly attend interdisciplinary team meetings and case conferences to share insights, discuss patient progress, and contribute to collective care strategies. Engage in ongoing professional development by staying informed about current research and best practices in integrated care and mental health therapy. Patient Population / Client Focus Our ideal candidates will work primarily with adults and older adolescents experiencing both mental health disorders and physical health challenges. You will provide compassionate support for patients struggling with conditions such as anxiety, depression, PTSD, substance use, and chronic health issues. Your role is pivotal in enhancing the overall quality of life for our clients by addressing psychological hurdles that often complicate their medical conditions. Qualifications A Master’s degree or higher in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution. A current and valid professional licensure as an LCSW, LMHC, or LMFT in New York State. Demonstrated expertise in managing mental health issues for individuals with co-occurring medical conditions. Exceptional verbal and written communication skills, facilitating clear and empathetic interactions with clients and healthcare professionals. Strong commitment to maintaining ethical standards and confidentiality in all client interactions. Proficiency in electronic health record systems and other teletherapy platforms used in clinical settings. Ability to thrive in an interdisciplinary team environment, promoting collaborative patient care. Preferred Experience Experience working in a community health or substance abuse clinic, providing supportive therapy for diverse populations. Knowledge of trauma-informed care practices and their application in therapeutic settings. Understanding of evidence-based therapies and the ability to implement these in group and individual settings. Compensation and Benefits The annual salary for this role ranges from USD 90,000 to 100,000, commensurate with experience and qualifications.
