Key Responsibilities\n

As a key member of our team, you will, under the supervision of the Social Work Manager:

• Conduct thorough bio-psychosocial evaluations to assess patient needs and identify treatment barriers.

• Develop and implement personalized treatment plans utilizing both individual and group therapy strategies, regularly analyzing treatment effectiveness.
\n
• Document all social work activities and patient interactions accurately in health records while ensuring adherence to departmental standards.
\n
• Provide education and resources to patients and families to foster understanding and adherence to treatment recommendations.
\n
• Serve as an advocate for patients, coordinating community resources that support their social and mental health needs.
\n
• Collaborate with medical and support staff, contributing insights on psychosocial factors impacting patient care to enhance treatment compliance and overall satisfaction.

• Engage in ongoing professional development through training opportunities relevant to your clinical responsibilities.
\n
• Stay updated on local resources to better assist patients with their unique needs.

Patient Population / Client Focus\n

The LCSW will serve a diverse patient population, primarily composed of individuals experiencing mental health issues related to substance use and other psychosocial challenges. Qualifications\n

The successful candidate will possess the following qualifications:

• Master’s Degree in Social Work from an accredited institution.

Compensation and Benefits\n
