Key Responsibilities\n

• Monitor and document patient progress using electronic medical records, facilitating effective communication with an interdisciplinary team.

• Perform appropriate reporting of any instances of vulnerability or suspected abuse in accordance with legal mandates.

• Utilize technology to identify patients at risk for readmission, implementing proactive measures and ensuring clinical handoffs are completed.

• Actively participate in care system processes aimed at preventing hospital readmissions through strategic planning.

• Provide timely resource information and support to address identified financial and social needs of patients.

• Other duties as assigned, engaging proactively in team objectives that enhance patient care delivery.

Qualifications\n

• A Master’s or Doctoral degree in Clinical Psychology, Counseling, or Clinical Social Work from an accredited institution.
