Position OverviewThe Registered Nurse (RN) provides comprehensive, patient-focused care in a home health setting. This role is responsible for assessing patient needs, developing and managing individualized care plans, and collaborating with physicians and interdisciplinary teams to ensure high quality outcomes. The RN also ensures compliance with all regulatory, clinical, and documentation standards while supporting patient safety and continuity of care.Schedule & Work DetailsCoverage Area: King County and Snohomish County, WASchedule: Monday through Friday, 8:00 AM to 4:30 PMOn Call Requirement: Yes, includes 5 weekdays and 2 weekends minimumPay Rate: $51 to $53 per hourMileage Reimbursement: Yes, between patient visits from first to last visitProductivity Expectation: Minimum of 30 productivity points per weekFlexibility: Required for weekend rotation and potential holiday coverageKey ResponsibilitiesClinical CompetenceInitiate, develop, implement, and revise individualized plans of care in collaboration with physicians and interdisciplinary teamsPerform initial and ongoing nursing assessments to determine patient eligibility and medical necessitySupervise and direct care provided by home health aides and LPNs, including task delegation and required supervisory visits in compliance with regulationsDocumentation and Care DeliveryProvide skilled nursing care in accordance with the plan of care and infection control standardsComplete OASIS assessments accurately and timely, including medication reconciliationMaintain accurate, thorough, and timely documentation in accordance with agency and payer requirementsEnsure all documentation is synced promptly per agency policyQuality and Care CoordinationConduct initial and comprehensive evaluations, ensuring compliance with payer and state-specific guidelinesDevelop and coordinate care plans with physicians and care teamsCommunicate effectively with internal staff regarding patient needs, scheduling, and care delivery updatesCoordinate care with clinicians, physicians, patients, and families to ensure continuity and quality outcomesParticipate in efforts to reduce hospitalizations and improve patient safety and outcomesProfessionalism and TeamworkMaintain a positive attitude, professionalism, and willingness to support patients and team membersAttend and actively participate in meetings, including case conferences and training sessionsFollow direction from leadership and complete assigned tasks promptlySupport onboarding and training of new staff and serve as a preceptor when neededParticipate in survey readiness, quality improvement initiatives, and utilization management processesMaintain schedule flexibility to meet patient care demandsRequired QualificationsActive, unencumbered Registered Nurse license in Washington State or compact multi-state licenseCurrent Basic Life Support (BLS) certificationStrong clinical assessment and care planning skillsAbility to work independently and collaboratively in a home health environmentPreferred QualificationsPrior home health experienceExperience with HCHB or PointCare systemsStrong communication, organization, and time management skillsExperience completing OASIS documentationPride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, pet insurance and employee discounts with preferred vendors.
