Key Responsibilities\n

• Conduct clinical social work evaluations to identify client needs and develop personalized treatment plans in collaboration with interdisciplinary team members, including psychiatrists.

• Deliver therapeutic counseling and ongoing support for clients, addressing their emotional and behavioral challenges through one-on-one sessions and group therapy.

• Manage an ongoing caseload by creating and maintaining comprehensive documentation pertaining to treatment goals, progress, and discharge planning, adhering strictly to company policies.

• Implement crisis intervention strategies effectively to support clients experiencing urgent mental health episodes.
\n
• Facilitate pre-release planning, assisting clients transitioning out of treatment programs to ensure a seamless reintegration into their daily lives.
\n
• Engage in case management by coordinating care across multiple services, conducting utilization reviews, and participating in regular treatment team meetings.
\n
• Research, connect, and advocate for community resources that promote client wellbeing, including support for access to food assistance, childcare, and healthcare services.

Patient Population / Client Focus\n

In this role, you will work with a diverse clientele, including individuals who face a range of mental health issues, traumas, and socioeconomic challenges. Our patient population consists of adults and adolescents seeking assistance for conditions such as anxiety, depression, PTSD, and substance use disorders. Qualifications\n

• A Master’s Degree in Social Work from an accredited graduate program.

• Active and unrestricted California Clinical Social Worker License (LCSW).
