Key Responsibilities\n

• Conduct comprehensive psychosocial assessments that consider emotional, mental, and social determinants affecting seniors’ well-being.

• Collaborate closely with clients and their families to develop tailored care plans that address individual goals and preferences.

• Provide counseling services, offering emotional support and guidance to seniors as they navigate complex life transitions, such as health declines or loss of loved ones.

• Advocate on behalf of clients to ensure they access essential community resources and services, promoting their rights and well-being.

• Maintain regular check-ins and visits to assess client progress, safety in their living environment, and to address emerging concerns.

• Document all interactions and assessments meticulously in our Electronic Health Record (EHR) systems to ensure continuity of care.

The primary focus of this role is on seniors accessing our health clinic services. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Valid and current state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

The compensation for this role ranges from USD 60,000 to 100,000 annually, commensurate with experience and qualifications.
