Work Environment\n

The LCSW role is entirely remote, allowing you to create a comfortable workspace while helping clients across various settings. While most interactions occur virtually, some flexibility is required for evening and weekend hours to meet the needs of our clients. As part of our multidisciplinary team, you'll collaborate closely with fellow healthcare professionals, including psychologists, primary care providers, and hospice staff to implement holistic treatment strategies.

Key Responsibilities\n

• Utilize your clinical training and experience to develop independent psychosocial assessments and diagnoses for patients navigating life-limiting illness and end-of-life issues.

• Engage with clients to co-develop personalized treatment plans that are centered on their unique psychosocial needs and goals.

• Implement a variety of evidence-based treatment modalities, including solution-focused therapy, motivational interviewing, and behavioral activation, consistently evaluating their effectiveness.

• Work collaboratively with other mental health practitioners and service providers to deliver cohesive care plans and ensure a seamless experience for patients and families.

• Monitor patient progress through regular follow-ups, both virtual and in-person, tracking treatment responses in alignment with established objectives.

• Facilitate medication management by coordinating with medical providers to ensure adherence to psychotropic medications while assessing for side effects and overall efficacy.

Patient Population / Client Focus\n

Our LCSW will work primarily with terminally ill patients and their families, facilitating psychosocial support while addressing grief, anxiety, and emotional distress that are commonly experienced during this challenging phase of life. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
