Case Manager Behavioral Health

Day Shift, 5 x 8s, 8:00 AM - 4:30 PM, no set days of the week.

Guaranteed Hours: 40.00 hours/week Contract Weeks: 13

Bachelor's Degree in Nursing (BSN): Preferred

Registered Nurse (RN) licensure in the state of practice: Required

At least two years recent experience in field

Current BLS certification

Epic experience

Behavioral health background: Preferred

Case management certification: Preferred

Lead the coordination of patient care with other disciplines within the care team, monitoring the appropriateness and timeliness of care.

Ensure the interdisciplinary care plan aligns with the patient's clinical course, continuing care needs, and covered services.

Discuss the appropriateness of resource utilization, consultations, treatment plans, estimated length of stay, and discharge plans with physicians.

Focus on managing complex patients and frequent emergency department utilizers.

Coordinate the transfer of patients to tertiary centers, including essential patient information for continuity of care.

Provide oversight and collect data for regulatory and accreditation compliance.

Manage frequent emergency department visitors by conducting focused reviews of previous and current admissions.
