Work Environment

This role is primarily set in the comforting environment of patients' homes, allowing for personalized and focused therapeutic interventions. Key Responsibilities

• Evaluate patients' physical conditions, utilizing comprehensive assessment techniques to devise tailored treatment strategies based on physician referrals.
• Implement effective therapy interventions using techniques and modalities appropriate for home care settings, addressing issues related to orthopedic, pediatric, neurologic, and sports medicine.
• Maintain superior customer service standards, ensuring a positive experience for all patients and their families as part of our commitment to care excellence.
• Work cohesively with a multi-disciplinary team to leverage shared knowledge and optimize treatment plans for patients.
• Utilize various therapeutic modalities and exercises to achieve predetermined functional goals, based on the prescribed plan of care.
• Educate patients and their caregivers on the therapeutic process, self-management techniques, and preventive care strategies.
• Monitor patient progress, document treatment outcomes, and report to referring physicians to continuously adjust care plans as necessary.
• Meet established business objectives, including productivity and patient retention goals, to contribute to the organization's operational success.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution, preferably from a program accredited by the American Physical Therapy Association (APTA) or the Commission on Accreditation in Physical Therapy Education (CAPTE).
• A valid state license to practice physical therapy in New York.
• Current Basic Life Support (BLS) Certification from the American Heart Association.

Compensation and Benefits
