Work Environment

This position is primarily onsite and involves providing physical therapy services directly in patients' homes. Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities, limitations, and medical histories to devise effective individualized treatment plans.
• Implement therapeutic interventions designed to improve mobility and alleviate pain, utilizing methods such as manual therapy, exercise, and specialized equipment.
• Monitor and document patient progress meticulously, adapting treatment methods and plans as necessary to achieve optimal results.
• Educate patients and their families about exercises, techniques, and lifestyle modifications to support rehabilitation goals.
• Ensure compliance with patient confidentiality and all legal/ethical regulations related to patient care.
• Engage in continuous professional development and participate in institutional training sessions to enhance skills and knowledge.
• Collaborate effectively with other healthcare professionals to coordinate patient care and contribute to multidisciplinary teams focused on holistic care.
• Maintain a clean, safe, and hazard-free working environment, addressing any unsafe conditions promptly.

Patient Population / Client Focus

The Physical Therapist will work with a diverse array of patients, including individuals recovering from surgery, those with chronic pain conditions, and older adults requiring assistance with mobility. Qualifications

• Must hold a valid Physical Therapist license in the state of New Jersey.
• A Bachelor's degree in Physical Therapy or a related field is required; advanced degrees are preferred.
• Current American Heart Association CPR/BLS certification is mandatory.
• Strong understanding of federal and state regulations concerning physical therapy practices.
• Excellent written and verbal communication skills for effective patient education and documentation.

Preferred Experience

• A minimum of one year of experience in a clinical setting is preferred, but new graduates are also encouraged to apply and will be considered.
• Experience with home care physical therapy is advantageous but not mandatory, as training will be provided.
• Familiarity with electronic health records (EHR) systems is a plus.

Compensation and Benefits
