In this full-time role, you will play a crucial part in delivering superior physical therapy services within a home care setting, ensuring that our patients receive personalized and effective treatment while experiencing the comforts of their own environment.

Work Environment

This position offers a unique work setting as you will primarily provide care onsite at patients' homes throughout Forest Hills and surrounding areas. Our approach focuses on fostering a collaborative and supportive team environment, where each team member is valued and contributes to enhanced patient outcomes. You will work Monday through Friday, from 9 AM to 5 PM, managing a balanced patient load of up to seven clients per day, allowing you to give each patient the attention and quality of care they deserve.

Key Responsibilities

• Conduct comprehensive patient assessments to develop individualized treatment plans based on specific needs, medical history, and therapy goals.
• Utilize evidence-based practices and innovative rehabilitation techniques to implement treatment strategies that facilitate recovery and mobility enhancement.
• Educate and motivate patients on their conditions, ensuring that they understand their treatment progress and the importance of adherence to their therapy programs.
• Monitor and document patient progress accurately, adjusting treatment plans as necessary to achieve optimal outcomes.
• Collaborate with other healthcare professionals and caregivers to ensure a holistic approach to patient care.

Patient Population / Client Focus

Your primary focus will be on an outpatient client base requiring rehabilitation due to various conditions, including post-surgical recovery, orthopedic injuries, and neurological disorders. You will have the opportunity to work closely with patients of all ages, tailoring your therapy approaches to meet their diverse needs and helping them regain their independence.

Qualifications

• A valid license to practice as a Physical Therapist in New York.
• A Bachelor's degree in Physical Therapy, with a preference for candidates holding a Master's or Doctorate degree in the field.
• A minimum of two years' experience in an outpatient physical therapy setting, providing direct patient care.

Preferred Experience

• Experience with home care physical therapy, specifically in developing and implementing rehabilitation plans in non-clinical environments.
• Familiarity with electronic health records (EHR) systems for accurate documentation and patient management.
• Strong interpersonal skills, with the ability to build meaningful relationships with patients and their families.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 per year, commensurate with experience, alongside a comprehensive benefits package.
