Work Environment

This position is rooted in a home care setting, allowing you to work directly with clients in their own environments, which can significantly enhance the therapeutic process. You'll conduct in-person evaluations and treatments, developing individualized plans that consider each client's unique home circumstances. Given the evolving nature of healthcare, flexibility and adaptability are crucial as we may also utilize telehealth services when clinically appropriate. You will collaborate closely with other healthcare professionals within the organization to provide comprehensive care.

Key Responsibilities

• Conduct thorough physical therapy evaluations for children and adults, assessing their physical abilities, limitations, and needs.
• Develop, implement, and adapt individualized treatment plans that align with clinical best practices and patient goals.
• Regularly reassess patients to monitor progress, adjusting treatment plans as necessary based on clinical insights and patient feedback.
• Document all patient interactions, evaluations, and progress using the company's electronic health record system to ensure timely and accurate record-keeping.
• Engage with patients and their families to educate them about the recovery process, setting realistic expectations and providing emotional support during their rehabilitation journey.
• Collaborate with colleagues in various therapeutic disciplines to ensure integrated service delivery for clients receiving multiple forms of therapy.

Patient Population / Client Focus

Your responsibilities will span a diverse demographic of clients, including children and adults with varying physical disabilities, injuries, or conditions. Qualifications

• A Bachelor's degree in Physical Therapy is minimum required; a Master's degree or Doctor of Physical Therapy (DPT) from an accredited program is preferred.

• Experience with both in-person therapy and telehealth practice is highly desirable.

Compensation and Benefits
