Key Responsibilities\n

Your primary duties will encompass a variety of counseling activities designed to address the mental and emotional well-being of patients. Your key responsibilities include:

• Conduct comprehensive patient intakes focusing on diagnostic and functional evaluations, providing insight and recommendations to primary care providers regarding treatment plans.

• Provide brief, focused interventions for patients in need of mental health services, ensuring immediate support and care.
\n
• Collaborate with the primary care team to identify patients who may benefit from specialty mental health services.
\n
• Develop and implement follow-up strategies to monitor patient progress and adjust treatment plans as necessary.

• Assisting primary care providers by offering educational insights into behavioral health care and strategies for managing mental health conditions alongside physical health issues.

• Document patient progress and maintain accurate clinical records, adhering to ethical and organizational standards.

Patient Population / Client Focus\n

The clients you will serve range widely in age and background, encompassing individuals facing mental health challenges, families experiencing social issues, and those recovering from trauma or dealing with chronic illnesses. Qualifications\n

To be considered for this position, candidates must meet the following qualifications:

• A Master’s Degree in Clinical Counseling or a related field is required.

• Current LPC (Licensed Professional Counselor) license from an accredited institution.
