Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, observances, and thorough reviews of medical records to identify the emotional and social needs of patients.

• Develop, implement, and monitor individualized treatment plans by collaborating with our rehabilitation team, addressing each patient’s unique psychosocial challenges.

• Provide counseling and therapeutic support to patients and their families, focusing on adjustment issues and emotional coping mechanisms during the recovery process.
\n
• Act as a resource to assist patients in accessing vital community resources such as housing assistance, vocational rehabilitation, and support groups that align with their recovery goals.

• Engage actively in discharge planning to ensure a smooth transition for patients returning home or moving to other care settings, thereby preparing them for sustained recovery.

Patient Population / Client Focus\n

In this role, you will work predominantly with individuals experiencing substance use disorders and various behavioral health challenges. Our diverse client population includes adults of varying ages and backgrounds, all seeking recovery support. Your role will significantly contribute to their journey toward sobriety, emotional health, and social reintegration.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Preferred Experience\n

Compensation and Benefits\n
