Our Values Put Patients First Empower Entrepreneurial Provider and Care Teams Operate with Integrity & Excellence Be Innovative Work As One Team What You'll Do Diagnose, interpret test results and treat common acute and chronic pediatric illnesses, applying evidence-based medical practices Develop and implement personalized treatment plans for patients, considering their medical history, family dynamics, and individual needs Collaborate with the patient’s family to create and execute treatment plans and ensure proper coordination of care to optimize patient’s health progress Maintain patient Electronic Medical Record data Refer to specialists when appropriate Collaborate with regional and central leadership to meet health quality and clinical excellence goals Other duties as assigned Qualifications Unrestricted and active California Physician Assistant or Nurse Practitioner License Current BLS, ACLS, CA DEA certification prior to start date EMR proficient Proficiency in Cantonese, Mandarin, or Spanish You're great for this role if you have: Prior experience working in pediatrics Experience working in a collaborative setting to ensure positive patient health outcomesInterest in working with Medicare patients Knowledge of Value-Based Healthcare model Environmental Job Requirements and Working Conditions This position is located at 328 S 1st Street, Alhambra CA 91801. The total compensation target pay range for this role is between $15,000 - $160,000 per year. This pay range represents our national target range for this role. Actual compensation will be determined based on geographic location (present or future), experience, and other job-related factors. This is a full-time position.
