Why You'll Love This Opportunity Help build and shape a growing Field Medicine program. Enjoy a hybrid role combining outpatient primary care in the clinic with community-based care in supportive and temporary housing. Collaborate with case managers, behavioral health providers, and social service teams to deliver truly integrated care. Make a meaningful difference for patients experiencing homelessness, behavioral health conditions, substance use disorders, and other complex medical and social needs. No street encampment outreach - patients are seen in supportive and temporary housing locations. Supportive physician leadership and a collaborative, mission-driven culture. Schedule Monday-Friday 1-2 days per week providing care at Field Medicine sites, with the remaining days spent delivering outpatient primary care at the North Hollywood clinic. One paid Saturday clinic approximately once per quarter, based on availability. What You'll Do Provide comprehensive outpatient primary care for adult patients in both clinic and community settings. Diagnose and manage acute and chronic medical conditions using evidence-based practices. Collaborate with multidisciplinary teams to coordinate medical, behavioral health, and social services. Educate patients on disease prevention, wellness, and chronic disease management. Review labs, manage referrals, coordinate follow-up care, and participate in quality improvement initiatives utilizing the Patient-Centered Medical Home (PCMH) model. Serve as a trusted advocate, helping patients navigate complex healthcare and social service systems. We're Looking For Someone Who Has a strong outpatient primary care background. Is passionate about caring for underserved and vulnerable populations. Is comfortable working independently while collaborating with multidisciplinary care teams. Has experience caring for patients experiencing homelessness, behavioral health conditions, substance use disorders, anxiety, and depression. Is resilient, resourceful, personable, and thrives in environments where every day is different. Has experience in an FQHC and/or Patient-Centered Medical Home (PCMH) setting (preferred). Bilingual Spanish is a plus. Qualifications Active and unrestricted California license. Current DEA registration and BLS certification (as applicable). Valid California driver's license. Experience documenting patient care in an electronic health record (EHR). $170,000 - $185,000 / year Additional compensation for quarterly Saturday clinics.
