Work Environment

This position is fully onsite, with all services delivered within the patients' homes. As a home care therapist, you will experience diverse settings and patient needs, allowing you to develop a broad skill set in varied environments. You will collaborate closely with caregivers, families, and other healthcare professionals while remaining flexible and mobile in delivering care.

Key Responsibilities

• Conduct thorough evaluations of patients in home settings to determine their physical therapy needs and develop comprehensive treatment plans.
• Utilize a range of therapeutic techniques, including manual therapy, functional exercises, and assistive devices to help alleviate pain and improve mobility.
• Regularly document treatment processes, patient responses, and progress to ensure continuity of care and adjust treatment plans as necessary.
• Educate patients and their families about rehabilitation techniques, exercises, and the correct usage of assistive devices to enhance recovery.
• Participate in ongoing training and professional development to stay current with the latest therapy techniques and best practices.
• Collaborate with other healthcare professionals to create holistic treatment strategies focused on patient well-being.

Patient Population / Client Focus

This role will include working with a diverse patient population, including children, adults, and older adults with various conditions such as post-surgical recovery, sports injuries, neurological disorders, and chronic pain syndromes. Our goal is to empower each patient to achieve their highest level of functional independence and quality of life in a familiar and comfortable environment.

Qualifications

• Doctorate Degree in Physical Therapy (DPT) from an accredited program.
• Current, valid Florida State License to practice Physical Therapy.
• Excellent verbal and written communication skills to inform and educate patients and families effectively.
• Proficiency in utilizing digital documentation systems for patient management and progress tracking.
• Strong interpersonal skills and a patient-centered approach to therapy.
• A flexible attitude towards work hours and patient schedules is essential for this home care role.

Preferred Experience

• A minimum of 2 years of experience as a licensed Physical Therapist is preferred, although new graduates may be considered.
• Experience in home health care settings or with home therapy services would be advantageous.

Compensation and Benefits
