The position of Licensed Professional Counselor will be primarily remote, utilizing telehealth technology to provide counseling services. Key Responsibilities\n

• Collaborate with primary care providers to identify, address, and manage mental health and psychosocial concerns, recognizing your essential role within the integrated healthcare team.

• Guide primary care providers in determining which patients require specialized mental health support and which can continue receiving care within primary healthcare settings.

• Create and implement relapse prevention plans, assisting patients in maintaining stability and promoting long-term recovery.

• Identify “at-risk” patients and collaborate with them to develop proactive plans that mitigate the risk of escalating mental health issues.

• Facilitate and coordinate behavioral health services, establishing supportive connections within the primary care framework and, with patient consent, extending resources into their communities.

• Document patient progress and vital diagnostic information accurately in treatment charts, maintaining thorough records required for ongoing evaluation and care.

• Maintain ongoing communication with primary care providers to ensure a cohesive understanding of patient needs, promoting collaboration on treatment strategies.

• Educate patients about self-management techniques and provide them with the tools they need to take an active role in their treatment and recovery journey.

• Collaborate with clinic leadership regarding behavioral health initiatives, contributing your expertise to improve service outcomes and program effectiveness.

• Support patient adherence to medical treatments initiated by primary care providers, including strategies to manage potential side effects of prescribed medications.

• Uphold the highest ethical standards by adhering to the Code of Ethical Conduct for Clinical Professionals.

Patient Population / Client Focus\n

You will work primarily with veterans and their families, providing culturally competent care that acknowledges their unique experiences and challenges. Qualifications\n

• A Master's Degree in Clinical Counseling or a related field is required.
