Key Responsibilities\n

• Conduct comprehensive assessments to identify patient needs and develop personalized treatment plans.

• Engage in regular communication with supervisors, colleagues, family members, and guardians, providing updates on patient progress.

• Prepare and manage a schedule of appointments, follow-up visits, and necessary home evaluations to ensure effective monitoring of patient growth.

• Implement and oversee program plans, making adjustments based on the continuous needs of clients.
\n
• Contribute to Interdisciplinary Team Meetings, sharing insights and collaborating on patient care strategies.
\n
• Document and complete monthly and quarterly progress notes to track patient development.
\n
• Organize and facilitate training programs to ensure all staff are proficient in program details and documentation standards.
\n
• Conduct follow-up interviews and ensure ongoing support to meet the evolving needs of individuals.
\n
• Maintain comprehensive records, ensuring accuracy and accessibility of patient information.

• Serve as a liaison with external mental health networks to align organizational goals and support community access to mental health resources.

Patient Population / Client Focus\n

Your role will primarily involve counseling individuals from various backgrounds, including adults and adolescents facing mental health challenges. You will provide tailored support addressing issues such as anxiety, depression, trauma, relationship difficulties, and substance misuse. Qualifications\n

• A Master’s or Doctorate degree in Counseling, Psychology, or a related field from an accredited institution.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of New Jersey.

• A minimum of two years of professional counseling experience, specifically in a clinical setting.

• Strong knowledge of ethical and legal standards within the mental health field.

• Excellent interpersonal and communication skills, with the ability to build rapport with diverse clientele.

• Proven experience working collaboratively with healthcare teams to enhance patient care.

• Familiarity with electronic medical record (EMR) systems and other telehealth technologies.
