Key Responsibilities\n

• Collaborate closely with primary care providers to identify and manage mental health and psychosocial issues, contributing to a seamless care team approach.

• Conduct patient intakes with an emphasis on diagnostic evaluations, providing insightful recommendations for treatment goals and plans based on established guidelines.

• Deliver brief, focused interventions for patients requiring mental health services, ensuring timely responsiveness to their emotional and psychological needs.

• Provide ongoing assessments to determine the appropriate level of care, advising on referrals to specialty mental health facilities when necessary.

• Initiate and facilitate follow-up interventions to monitor patient progress and adjust treatment approaches as needed.

• Develop individualized relapse prevention plans to help patients maintain stability and enhance their quality of life.

• Identify at-risk patients and implement strategic plans to address their vulnerabilities before they escalate into more serious issues.

• Document patient progress and diagnostic information with accuracy, ensuring all records are maintained for compliance and continuity of care.

Compensation and Benefits\n

The position offers a competitive salary ranging from USD 87,000 to 105,000 annually, commensurate with experience and credentials.
