Key Responsibilities\n

• Conduct thorough evaluations and assessments of children’s developmental needs.

• Participate in the Individualized Family Service Plan (IFSP) development, collaborating with families to create tailored support strategies.

• Implement the IFSP for enrolled families, including recommending suitable service frequency based on developmental progress and family needs.

• Monitor and document outcomes as an active member of the multidisciplinary team.
\n
• Provide coaching and support to families using evidence-based practices that foster natural learning environments.

• Develop rapport with children and families to create a nurturing learning atmosphere conducive to growth and development.

• Ensure compliance with all federal and state regulations while promoting health and safety standards for all individuals served.

Patient Population / Client Focus\n

The primary focus of this role will be on infants and toddlers who are experiencing developmental delays or disabilities. Qualifications\n

• Master’s Degree or Doctorate in Social Work, with current LCSW licensure recognized by the Department of Public Health.
