Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, behavioral observations, and thorough reviews of medical records to identify emotional, social, and environmental factors affecting our patients' recovery.

• Offer compassionate counseling and emotional support to patients and their families, assisting them with adjustment issues and overall mental wellness throughout the rehabilitation process.

• Work collaboratively with a multidisciplinary rehabilitation team to develop, implement, and monitor individualized treatment plans that incorporate the patients' psychosocial needs and rehabilitation goals.

• Guide patients and their families in navigating community resources, including but not limited to support groups, vocational rehabilitation programs, and logistical housing assistance.

• Participate actively in discharge planning to facilitate smooth transitions from rehabilitation back to home or alternative care settings, ensuring continuity of care and support.

• Provide ongoing support and education to patients and families regarding available post-rehabilitation services and community resources, empowering them for future success.
